ANSWER
The internet is full of "standard" license agreements, which must always be adapted to accomplish what you want. But licensing is complicated and not something you should try yourself, except perhaps when you are licensing yourself (i.e., when you have created a company you own, and you license the patent you own to this company). In such a case, you can modify the agreement later if you find it doesn't serve your needs. Any experienced intellectual property attorney can help you with the preparation of a license.
